✋🏼CAUSE vs EFFECT🤚🏼⁣⁣ ⁣⁣ True ☝🏼⁣⁣ ⁣⁣ Say for example you have lateral epicondylitis 💪🏼 (tennis elbow). We could throw all the various treatment at it - dry needling, heat, soft tissue and trigger point massage, mobilisation with movement, lateral or medial glides, splint, tennis elbow strap and the list goes on.⁣⁣ ⁣⁣ But if the cause of your symptoms remains, all of the above will likely have very little effect. If the reason for your symptoms is the way you pick something up at work and you continue to do this, chances are things will take a long time to get better.⁣⁣ ⁣⁣ Even though it can be frustrating to avoid a certain task in your work, sport or home life - taking the advice of your therapist and modifying your activities can have a positive effect on your rehab 👍🏼⁣⁣ ⁣⁣ 📸 metatherapy

🍌 RADIOLOGY 🍌 ⁣ ⁣ Love this image! It sums radiology up perfectly.⁣ ⁣ Often, if you have an acute injury with a forceful mechanism of injury you will be referred for an X-ray. This looks at your bones and joints and will confirm or rule out a fracture. ⁣ ⁣ An ultrasound is less common and looks at your soft tissues. Soft tissues can take just as long, if not longer, than a fracture to heal. We can refer you for an X-ray or ultrasound if indicated. ⁣ ⁣ A CT and MRI are the next level up in diagnostic imaging. In NZ, a specialist will refer you for these tests if needed. ⁣ ⁣ 📸 healthplus_imaging

🤜🏼 BOXERS FRACTURE 🥊 ⁣ ⁣ Also known as a 5th metacarpal neck fracture. As the name suggests, it often occurs after a closed fist comes into contact with a solid object.⁣ ⁣ ACC is a ‘no-fault’ system. That means if you have an accident, whatever the cause, your treatment and rehabilitation is covered.⁣ ⁣ We hear all sorts of stories about how an injury occurred. Telling your therapist the correct mechanism of injury is important. No judgement here 👍🏼 👊🏼⁣

☺️ OUR STORY SO FAR ☺️⁣ ⁣ By now you probably know a bit about us. A quick recap⁣ ⁣ We have 2x Auckland Hand therapy clinics ✌🏼⁣ ⁣ AIRPORT OAKS - located within a busy medical centre, we have access to a GP and Nurse if required. This has come in very handy when dealing with nasty wounds. Airport is open 3 days per week, soon to be 4. ⁣ ⁣ GLENFIELD - located within a well established physio practice, this is a great supportive environment that we love. Glenfield is open 6 days per week, including Saturday AM.⁣ ⁣ Both clinics were set up to serve our local community and make access to hand therapy that much easier 👍🏼

Back and Shoulders Another good workout. Alternating between 10 rep sets and 12 rep sets. Today I did 12 instead of 10 for all sets On top of my regular workout in the gym, I've been keeping up with a bodyweight workout sometime in the same day. Don't know the science behind it, but it curbs the fatigue for those muscles and seemingly helps gains to happen sooner Decided I'd do back and front from now on. Funniest workout ever. Almost saw my dinner, about six times throughout my workout. Made the mistake of having Subway AND a Mass Gainer Shake not long before my workout. Pump wasn't as great as it has been in the past, but eh I've noticed that I haven't been doing my Bench Presses properly 😅 so this week, before I put more weight on, I'm practicing my form, so that the Bar touches my Chest, as well as positioning my Elbows so I reduce the strain on my Shoulders. Recently finished Rehab for my right shoulder Was listening to Arnold talking about getting a smaller waist on YouTube. Mentioned briefly that he did AT LEAST 200 Crunches a day, outside his regular workout. I've never been one for volume training my Core, but I've never done that before, so this week I'm trying it. My lower Core is where I lack, so I'll be doing 200 Reverse Crunches instead Monday Workout as follows: . CHEST Incline B. Bell Press SS (Push Ups) 10,10,10 (10,10,10) Decline B. Bell Press SS (Push Ups) 10,10,10 (10,10,10) D. Bell Pullovers 10,10,10 Cable Crossovers [move past center Chest, alt lead arm] 10,10,10,10 Machine Flyes 10,10,10 . ARMS Iso. Bicep Curls 10,10,10 Preacher Curls SS (Drag Curls) 10,10,10 (10,10,10) Rope Tricep Extensions SS (Bar Tricep Extentions) 12,12,12 (8,8,8) Skullcrushers 10,10,10 . CORE Cable Crunches 20,20,20 Vacuum Holds 15,15,15 (seconds) . Weighing in at 103kg flat now. Can't wait for 120kg. MEAN workout. Got a bit nervous last night because I was extremely sore after last night's workout, and I felt so fatigued that I felt physically weaker. 🦴 X-RAY 🦴⁣ ⁣ This is an X-ray of a ’s hand 🧒🏼 it looks like there are big gaps between the bones - but there aren’t! The ‘gaps’ are cartilage. These are areas that are yet to ossify or form hard, strong bone.⁣ ⁣ ’s xrays also show growth plates, at the ends of the bone. Growth plates are the last part of the bone to ossify and are prone to fractures. 15-30% of all childhood fractures are in the growth plate. For this reason, we see a lot of growth plates fractures amongst our patients 👎🏼

👉🏼SPLINTING TIPS👈🏼⁣ ⁣ ✖️ Use a paper towel to make your pattern on. Cheap, easy to use and you can recycle (or reuse to apply acetone)⁣ ⁣ ✖️ Measure down from the middle finger. This gives you a guide as to where the thumb hole should come to.⁣ ⁣ ✖️Use a pastel to mark the pattern onto the thermoplastic. It is easy to see and washes off in the splint pan.⁣ ⁣ ✖️Always fold over the distal palmar crease and thenar portions as soon as the splint comes out of the pan and is nice and warm.⁣ ⁣ ✖️ Stroke don’t poke the splinting material when moulding onto your patient. Nobody wants big finger marks on their splint.

🙋🏼‍♀️CAN I BE A HAND THERAPIST TOO? • Maybe 🤔 are you already a Physio or Occupational Therapist? That’s what you need as a baseline. • Then there are a few different pathways to becoming an NZ Registered Hand Therapist. One of them I’ll outline below 👇🏼 • AUT runs a Hand & Upper Limb 30 point paper that a lot of people take (it’s really good!). Then you need to complete another 15 point paper. After that there are clinical hours to complete, supervision, peer review and a letter of recommendation 📚 🤓 • Once you are registered there is a lot of professional development you need to keep up with. • So yes, do all of the above and you too can be a Registered Hand Therapist 👍🏼 get in touch if you have any questions.

👩🏻 A DAY IN THE LIFE OF THTG 👩🏼 What do we get up to on a daily basis? Today started with my alarm at 5:40am 😴😳 for an early morning gym class. Home to get the ready. Daycare/school drop off then in the clinic ready for the first patient at 9am. Living local is 👍🏻 Unpacking stock, admin, answering emails amongst patient visits. Then it’s home time for the morning in reverse (minus the early wake up!) Because we are based over 2 clinics we have a range of clever technology to keep us in touch throughout the day. Lots of admin at the moment being in the early days of setting up. There are so many cool things in the pipeline for 2019 👍🏼

🦴 FRACTURE MANAGEMENT🦴 Fracture = broken bone. There are various ways we can manage your fracture. It depends on what is fractured, if you had surgery or not, and how far down the track you are. These decisions are made in consultation with your specialist or GP. If you have had surgery, we will often get you moving quite early on which surprises a lot of people. It is important to move as soon as you can, to avoid your soft tissue tightening up. Even if you haven’t had surgery for say, a finger fracture, your therapist will likely get you moving after 2 weeks.

🤜🏼 SPLINT VS CAST 🤛🏼 There are benefits to both, although we tend to splint more than cast here at THTG. A splint is made of thermoplastic - we love orfitindustries due to its ease of use and variety in colour and thickness. We make a splint in around 10 minutes, depending on how complex it is (don’t time us!) A splint can be removed for hygiene and remoulded. We make soft casts when needed aswell. These can be ideal for , or anytime you don’t want the cast removed. As you can see if the video, there is slight give to the material. It doesn’t set as hard as traditional fibreglass. Your friendly Hand Therapist will decide what best suits your needs 👍🏼

🤔 WHAT IS HAND THERAPY 🤔 And what exactly IS a hand therapist? Is it the same as physio? And when does the physio start? We get asked these questions all. the. time. * A hand therapist could be a Physiotherapist . Or they might be an Occupational therapist. Both have done extensive additional training in hand therapy. * The ‘physio’ is replaced by hand therapy. We take care of all your rehab from day 1 to discharge. * A hand therapist will set goals with you to get you back to whatever you want - fishing, rugby, picking up your 👶🏼, picking up a 🍺, tying your 👟 laces, planting a 🌲, driving a 🚗 or even a 🚀 and make it happen 🦸‍♀️ You name it, we will help you achieve it 👍🏼

👩🏻‍💻WORKSTATION SETUP👨🏽‍💻 Having your workstation set up correctly is important, as most of us are working from mobile devices these days, which can be far from ideal! The relationship between your screen height, screen distance, desk height and chair height needs to be correct for an optimal working posture. Especially if you are on the computer for long periods. A few handy tips: 🖐🏼 don’t spend more than 50 minutes in a 1 hour period on your computer. 🖐🏼 position your printer away from your desk so you have to get up and move to retrieve any printing. 🖐🏼 your mouse settings can be adjusted if you need to change hands. The buttons are switched over so the index will still be the left click and middle finger the right. 🖐🏼 if you are experiencing arm, back or neck pain at work, a workstation assessment may be required. 📸 byrawpixel

🔧 GETTING BACK TO NORMAL 🤹🏼‍♀️ • Returning to your ‘normal’ life after an injury can be a balancing act ⚖️ Getting back to your pre-injury activities, whether it be sport, work or playing an instrument is a great form of rehab when your therapist gives you the all clear 👍🏻👍🏻 👍🏻👍🏻👍🏻👍🏻 PACING - if you normally spent 4 hours in the garden, start off in shorter blocks. Spread the time over the day and stop 🛑 BEFORE it gets sore or tired. • GRADING - If you used to lift 10kg at the gym 🏋🏻‍♀️ start back with much lighter weights and less reps than normal. Same as above applies - 🛑 before you get sore. Fatigue is normally a good indication you’ve had enough.

🤜🏼 F R A C T U R E S 🤛🏼 We need to clear something upa fracture and a break are the SAME thing 🙀 often people get a wee bit confused and think when they are diagnosed with a fracture, it is not as serious as a ‘break’. But it is, it’s exactly the same. Types of fractures include transverse, compound, comminuted, oblique and transverse just to name a few. So remember fracture = break 👍🏻
